Improper communication............

1. Always talk with the others members involved before starting any project.

2. Decide on common agenda and goals.

3. Seek proper written communication from time to time.

4. Dont blame others if caught, dare to take responsibility.

5. It seems very easy to build but sometimes almost impossible to implement.

6. Last, do whatever you wanna do but it happens very often :-) .
